60M with dizziness

 60 year old male from Nalgonda came to the opd with 

CHIEF COMPLAINTS

Dizziness and loose stools since 15 days 


HISTORY OF PRESENTING ILLNESS

patient was apparently asymptomatic 15 days ago then developed dizziness every alternate day associated with loss of Conscious.

Patient also complained of passing loose stools since 15 days, half an hour after having food. Not associated with blood, no history of Vomitings,tenesmus


PAST HISTORY 

history of trauma( fell off from a bike) then developed an episode of seizures 4 months ago.

DM since 8 years 

HTN since 8 years 

Not a k/c/o TB, asthma


PERSONAL HISTORY 

Diet:mixed

Appetite:normal

Sleep: inadequate( only one hour every day)

Allergies:Nil

Addictions:Nil


GENERAL EXAMINATION

Patient is c/c/c examined in a well ventilated and lit room with consent 

No pallor,icterus,cyanosis,clubbing,lymphadenopathy,pedal edema. 


Romberg’s sign negative 

No nystagmus


VITALS

BP- 140/80 mm of Hg

Temp- afebrile

Pulse rate- 80 beats/min

Respiratory rate- 20cycles/min
