32F with Fever

 


Hi, this is Shreshta Reddy Adla, a medical student currently studying in KIMS Narketpally. CHIEF COMPLAINTS 

Of fever  since 5days


HISTORY OF PRESENT ILLNESS 

Patient was apparently asymptomatic 5 days ago



then developed fever which was

insidious on onset

gradually progressive

intermittent

Associated with chills and rigors

high grade

 aggravated   in nights

Temporarily relives on medication 

No history of body pains ,head ache, Vomitings, diarrhoea.


PAST HISTORY 

Not a known case of 

HTN, DM, TB ,Asthma , epilepsy

PERSONAL HISTORY 

Sleep inadequate 

Appetite decreased 

No addictions 

No allergies regular bowel bladder movements 


FAMILY HISTORY 

Not relevant 


GENERAL EXAMINATION 

Patient is c/c/c and is examined in a well ventilated room with consent 

No pallor icterus cyanosis clubbing of fingers lymphadenopathy and peda edema
